 Electronic Key Card

Please complete the form below to request an electronic amenity/pool card key, activation of your card, or help with a card key problem. Your card key allows you and your family entry into the PHE HOA amenities. If your HOA dues are not current, your card key is deactivated. A replacement or additional card will also be deactivated until HOA dues are paid.

NOTE: If you paid your assessments on time and had a zero balance on your account as of January 25th or July 25th, you do not need to reactivate your cards. Only the accounts that were delinquent after those dates need to reactivate cards.  It is the homeowner’s responsibility to request card reactivation.

ACKNOWLEDGEMENT: I (the homeowner named above) acknowledge that a $10 fee is required to obtain each additional or replacement card key. The fee, if applicable, will be collected upon delivery of the card key(s).
